Please come to the Surgery and fill in a registration form and supporting documentation.
You will then be asked to attend for a new patient health check, which will include a brief medical history, blood pressure check, urine test, your height and weight and a review of your smoking status. This must happen within the first two months of registration. However, if for any reason you wish to decline this health check please let us know. Failure to attend or advise us accordingly will mean that we will assume you are no longer requiring registration and your notes will be returned to the Kent Primary Care Agency.
New patients on regular medications will need to be seen by a doctor before being issued with a repeat prescription. If you are a new patient registering with the Practice, please (if possible) allow us 48 hours after handing in your registration documentation to prepare your computer record etc, before making any appointments.
We will request your previous NHS notes from your previous GP Practice, via our Health Authority and this can take several weeks.
What do I do if I change my address or telephone number after registering?
Please complete a change of address/personal details form at the Practice. We may have to review whether you are still in our Practice area and if necessary advise you about how to register with another GP Practice.
You will appreciate how important it is that we have up to date address and telephone details in order to be able to contact you.
Why do you need so much information about me and where does it all go?
We ask for information, so that you can receive proper care and treatment.
We keep this information, together with details of your care, because it may be needed if we see you again.
We may use some of this information for other reasons: for example, to help us protect the health of the public generally and to see that the NHS runs efficiently, plans for the future, pays its bills and accounts for its actions.
Information may also be needed to help educate tomorrow’s clinical staff and to carry out medical and other health research for the benefit of everyone.
Sometimes the law requires us to pass on information: for example in the case of a birth.
The NHS Central Register for England and Wales contains basic personal details of all patients registered with a general practitioner. The Register does not contain any clinical information.
You have a right of access to your health records.
You may be receiving care from other people as well as the NHS, So that we can all work together for your benefit, we may need to share some information about you.
We will only ever pass on information about you, if people have a genuine need for it, in yours and everyone’s interests. The sharing of some types of very sensitive and personal information is strictly controlled by law.
Anyone, who receives information from us, is also under a legal duty to keep it confidential.
The main reasons for which your information may be needed are:
Giving you health care and treatment
Looking after the health of the general public
Managing and planning the NHS
Making sure that our services can meet patient needs in the future
Paying your doctor, nurse, dentist or other staff and the hospital, which treats you
Auditing accounts
Preparing NHS statistics
Investigating complaints or claims
Helping staff to ensure the care they provide is of the highest quality
Training and educating staff
Assisting in research, which has been approved by the local ethics committee
What are my responsibilities?
Please provide as much notice as possible if you are unable to attend for an appointment
Please understand that some patients may need a longer consultation time than the ten minutes, which has been allocated
Please try and avoid ringing before 10:00, unless your call is of an urgent nature
Please refrain from consuming food and drink, whilst on the surgery premises
Please note that smoking is forbidden in and around the surgery building
Please turn off your mobile phone, when entering the surgery building
Please note that the only animals permitted into the building are guide dogs
Please ensure that you keep us informed of changes to your personal details, such as name, address and telephone number
What are my rights as a patient?
To receive patient care of the highest standard

To know that the equipment used in the surgery will meet current safety standards
